40 yrs old women presented with acute abdominal pain. She denied excessive alcohol intake, takes no regular medication except the oral contraceptive pill and occassional analgesia for back pain. She gives history of DVT 5 yrs ago following a flight from Delhi. She does not smoke nor she had any previous abdominal surgery. On examination pulse 100/min BP 120/80 mm Hg. JVP not raisedand absent hepatojugular reflex. heart sounds were normal and no ankle edema chest was clear and abdominal examination revealed tender hepatomegaly with ascites. investigation revealed mild increase in APTT

what is her illness and cause.

This lady was having budd chiari syndrome hepatic vein thrombosis with tender hepatomegaly and ascites. Because of hepatic vein thrombosis patient had negetive hepatojugular reflex which also showed no evidence of cardiac failure. patient was having previous DVT and increased APTT patient is likely to have antiphospholipid antibody syndrome.
