A 52 years old male patient residing at North Gujarat area had been detected with pulmonary fibrosis since 2013, patient was never treated before 2016.

The patient is on steroid, doctor advised for tablet pirfenex (pirfenidone) but patient could not tolerate it due to its side effects like skin peeling, lack of hunger and suddenly stopped tablet pirfenex.

Now patient has breathlessness after walking or do any small work which did not happened till he was taking tablet pirfenex.

So what precautions or treatment should be continued for this disease??

First we should try to find out the cause of ild,like idiopathic or due to other autoimmune condition like scleroderma, rheumatoid arthritis,. in which immunosuppressant can be given

First with consultation with a radiologist please do confirm whether it is idiopathic pulmonary fibrosis. rule out (through history and relevant test) other causes of UIP pattern on HRCT (if that is the radiologist diagnosis). on starting pirfinedone ask the patient to cover him/herself and advice him not to go out in the sun ( due to photosensitivity) please do repeat his LFTs after a week before dose escalation.... No definite treatment available can try steroids, treat pulmonary hypertension and if breathlessness persist bipap can be tried
