A 56yr old male with chronic uncontrolled, asymptomatic hypertension 170/100mm/hg and Type 2 DM presents with sudden onset of dull pain in occipital region.
He is already on Telista trio cl 6.25, Metoprolol 50mg, Ecosprin 150mg and Glimepiride with Metformin. No dyslipidemia seen. ECG and ECHO: NAD

Can this august members suggest further course of treatment?

Need more information for 1) neurological findings, including funduscopy, 2) status of diabetes control, , 3) renal function. Meanwhile antihypertensive dose can be increased, first Telmisartan, then Amlodipine If headache continues despite BP control, I would suggest neuroimaging ( *MRI or CT scan)
