A 58 yrs Indian Male presents to emergency dept with complaint of sudden unconsciousness 3 hours before. Patient regained consciousness after 10-15 mins but was having low level of consciousness for which he was shifted to critical care and mechanically ventilated. He was having fever for 7 days and constipation for 10 days. Patient underwent thyroidectomy 18 years before & is on HRT (Thyroxine 175micg per day). Patient is a k/c of Bipolar disorder since 8 years with following medication: sodium valproate 300mg, carbamazepine 400mg, clozapine 50gm, fluoxetine 20mg, clonazepam .25mg. Patient's current issues are high grade fever persistently above 101°c & low level of consciousness. Blood and urine culture came out to be sterile. CT and MRI appears to be normal. Patient is on ionotropic support Norad @ 3mg/kg/h and continuous paracetamol infusion @200ml/hr. Labs: Hb 8.4, Tlc 18000, Dlc 90/10, plt 2 lacs, sodium 160, pot 3.6, urea 160, creat 2.7, bilirubin 0.9, sgpt 49, sgot 78, pt 22.6, inr 2.04, aptt 93.3, ALp 170, CK1841, trop i 0.045. Echo suggestive of pulmonary embolism otherwise normal. ECG normal. What is the cause of persistent high grade fever and decrease level of consciousness? Management of patient?
