A 69 years old lady has been a known diabetic on Inj insulin and OHAs for the past two decades. She also had high BP controlled with Amlodipine and telmisartan. She stays in an old age home alone.
In June 2017 she was brought to me with a diarrhoea of a month duration for which she had been treated adequately by the local doctors. It took me another month to control her diarrhoea and vomiting. During this period she hardly ate anything. Her blood work was normal in August.
3 days back she has been again brought to me with hypoglycemia and hypotension despite stopping all medicines including insulin, OHAs, antihypertensive for a month.
What is the reason for her present state? I have a hypothesis. Could the forced fasting have reduced her insulin secretion and got rid of her insulin resistance?
