A common but expensive mistake
There is a marked tendency after medical mishaps/accidents to write unusually long and at times defensive or explanatory notes. Such notes always give an impression of a cover-up. Following precautions have been advised by the Court:

-Ensure that even after a medical mishap/accident medical records are written in the same way as in the normal course and not otherwise

-Missing sheets or missing medical records always lead to an adverse inference against the doctor/hospital. It is, therefore, necessary not only to prepare medical records carefully but also to be equally careful in preserving them

-It is advisable that medical records must be computerized. The Indian Medical Council (Professional Conduct, Etiquette, and Ethics) Regulations, 2002, mandates that efforts must be made by all doctors in this direction
