A patient had a history of csom 8 years back n was operated den ,aftr 8 year he suffers from episodes of vertigo on turning to affected side which was followed by purulent ear discharge again....he took amoxyclav fr 6 week but doesnt work....whats the next step ?

I would suggest to go for CMRI of brain and ear ossicles to look beyond just CSOM for vertigo and to rule out intra Cranial Abscess causing vertigo and acting like a reservoir for pus discharge which may require exploration

It can be anything either from labrynthitis to posterior Fossa Abscess or simple CSOM recurrent, but requires complete evaluation as it' s recurring and long standing
