ECG Challenge: A Counterclockwise Rotation*

A 76-year-old woman is admitted to the hospital because of abdominal discomfort, nausea, vomiting, and abdominal distention. She is diagnosed with a small-bowel obstruction.

She stated that she had undergone a cholecystectomy several years ago, but has never had any other medical problems or other surgeries. After admission, a 12-lead ECG is obtained (Image attached).

What does her ECG show?

A. Normal sinus rhythm (NSR) with premature atrial complexes (PACs)

B. NSR with junctional escape complexes

C. NSR, Mobitz I and blocked PACs

D. NSR with sinus pauses

E. Atrial tachycardia with variable block
