When lumbar discs herniate, the nucleus pulposus usually herniates posteriorly or posterolaterally. Lombardi first reported the posterior epidural migration of lumbar disc fragments (PEMLDF) as a rare case in 1973. PEMLDF is often confused with other lesions that form in the posterior epidural space, such as malignancy, hematoma, and abscesses , making it difficult to make a definitive diagnosis of PEMLDF prior to surgery. In all reported cases of PEMDLF, magnetic resonance imaging (MRI) at the first visit showed posterior epidural lesions . We here report a case of PEMLDF diagnosed by discography prior to surgery, and discuss the use of discography in the diagnosis of PEMLDF.
