Menopause Mirage - A curious case of a silent thyroid in plain sight.
A 50 year old female patient complained of lethargy,fatigue,somnolence and weight gain. Along with that,her skin had become dry and she had visible hair loss. Her daughter-in-law claimed her to be lazy,irritable and depressed since a few months. The genral practitioner in their village diagnosed her to be having menopause as she stopped having menstrual bleeding before six months. She was advised to take long walks to control her weight,but was unable to do so due to pain in her knees. As there was no improvement in her condition,she was bought to Ahmedabad and the case was taken up by a physician in a hospital. The doctor asked them to carry out routine blood and urine tests along with thyroid function tests. He also observed a slight swelling of thyroid gland on empty swallowing. On examination,her vitals were normal. The patient had no significant family history, TFT results: T3 – 76 ng/dl ; T4- 3.50 µg/dl; TSH =11.0mU/L
She was confirmed as a patient of hypothyroidism and put on a treatment of Levothyroxine replacement therapy,
