A 64-year-old man diagnosed with a vitreous hemorrhage of unknown cause in his left eye did not seek follow up care until he presented to the emergency room 7 years later with left eye pain, proptosis, and no light-perception vision. T1-weighed magnetic resonance imaging (MRI) demonstrated a left intraocular tumor with extension into the orbit and tracking along the optic nerve to the orbital apex. Gross inspection of a vertical section of the exenterated orbit, and later light microscopy, confirmed that the strong MRI signal corresponded to that portion of melanoma with densely pigmented cells in the eye and in the optic nerve. The surgical margin of optic nerve was occupied by heavily pigmented melanoma cells.....
