A 71-year-old man presented with left eye pain and vision loss 6 days after tripping and hitting his face on a table. In hindsight, he realized that the mouthpiece of his vape pen had broken off but could not find it. Computed tomography scan revealed orbital floor fracture and orbital foreign body juxtaposed to the globe (Fig A). Examinations was notable for proptosis, ophthalmoplegia, afferent pupillary defect, and light perception vision. B-scan ultrasonography revealed a large suprachoroidal hemorrhage. Using malleable retractors and Adson forceps, the tip was removed from the entry wound in the inferomedial conjunctival fornix (Fig B, C).
