Detailed evaluation of the entire spectrum of aortic root disease is possible with three-dimensional (3D) datasets provided by computed tomographic (CT) angiography and magnetic resonance (MR) imaging. The objective of this article is to review the role of CT angiography and MR imaging in the diagnosis, surveillance, and pre- and postoperative evaluation of aortic root disease. Patients with several underlying conditions, including connective tissue disorders such as Marfan syndrome, bicuspid aortic valve, and Loeys-Dietz syndrome, are predisposed to aneurysmal enlargement of the aortic root with potentially catastrophic outcomes if left untreated.
