Recently seen a young male 20 years old presented with fever for 5 days and during the course of admission developed backache with bilateral lower limb weakness and urinary retention with band like sensation in upper abdomen. acute transverse myelitis was suspected and patient was started on steroids. by the time mri and csf report came patient went altered. he was suspected to be possibly a case of acute demyelinating encephalomyelitis. reports stunned us; as we found it to be a case of potts spine with epidural abcess with disseminated cns tuberculosis. steroids were continued and patient was started on akt and mannitol, finally he is improving. just highlighting the importance of taking each fever seriously and why radiological help can make a huge difference.
