A 78 years old female brought up to the ICU in the condition of peripheral muscle weakness with Quadriparesis. Her history was diarrhoea before 2 days & sudden vertigo when she brought up to the hospital. Ataxia & Areflexia are found on examination with 140/80 mmHg of blood pressure. Blood examination & x ray done. Reports are as follow:

What should be the proper diagnosis and treatment?

GBS (guillain Barre syndrome)is an autoimmune disease in which there is damage to peripheral neurons and damage their myelin insulation (acute polyneuropathy).So there are problems related to motor / sensory or both neurons.So there are sings of ataxia,quadroparesis , areflexia, peripheral weakness,planter reflex decrease are positive! Symptoms develop over HOURS to a few WEEKS.During acute phase,the disorder can be life threatening with about 15\% of developing weakness of breathing muscles requiring mechanical ventilation. CAUSE : c.jejuni bac which cause diarrhea may some time influenza virus surgery, INFECTION, vaccination. DIAGNOSTIC TESTS : nerve conduction study examination of CSF(increase wbc n protein ) presence of antibodies. TREATMENT : immunoglobulins supportive care IV blood exchange. (these may take weeks to years to treat) once the weakness has stopped progressing,it persist stable level (plateau phase)before improvement occur.Level of CONSCIOUSNESS is unaffected! ... Read more

CSF report : increase protein with NIL total count,RBCs, lymphocytes, polymorphs,any organisms! Fluid was colourless,clear without turbidity.(3ml)

@Dr Satish,In Botulism,there is bulbar palsy (facial muscles paralysis)but here in this patient,chewing,eye movements, swallowing, facial expressions,vision(,no dropping of eyelids) are normal earlier.talking was normal earlier.so here it is NOT DESCENDING paralysis.Here it is ASCENDING paralysis.so your another option GBS is CORRECT in which there is ASCENDING paralysis!????
