The adrenocortical carcinoma is a rare aggressive endocrine malignancy that presents itself as an abdominal mass, symptomatic or incidental, often locally advanced or metastatic disease. Surgical resection when possible is the treatment of choice and margin-free resection is the most important prognostic factor for long survival.In this paper, we describe the case of a woman of 58-year-old, bearer of an enormous androgen-secreting cancer, which has gone unnoticed for years, presenting with abdominal hemorrhagic syndrome which required surgical resection in emergency.
