A 22-year-old woman presented with sudden loss of vision (no light perception) in her left eye 1 hour after injection of hyaluronic acid gel into her forehead. Examination disclosed complete ptosis and edema of the left upper eyelid, and ophthalmoplegia (Fig A); conjunctival erythema and corneal edema (Figs B–C); a dilated pupil, intraocular pressure of 6 mmHg, and retinal edema.

Angiography demonstrated absence of the left ophthalmic artery (Fig D, arrowhead) and magnetic resonance imaging showed extraocular muscle enlargement and cerebrospinal fluid pooling around the optic nerve (Figs E–F). The findings were attributed to retrograde embolization of hyaluronic acid gel into the left ophthalmic artery.
