First find out the cause whether it is Urge incontinence where there is increased frequency or overflow incontinence where there is difficulty in passing urine and patient has incontinence once bladder becomes full....Urge indicates UMN lesion and Overflow indicates LMN type of lesion...also possibility is Stress Incontinence which is seen after pelvic floor surgeries....agents such as solifenacin,darifenacin and tolterodine can be used...Solifenacin is available in doses of 5mg which can be given BD to TID... Its important to evaluate and classify the type of incontinence. Male' s and females have different etiologies. Age group specific causes are different. Broad evaluation would involve assessing bladder volume and functional efficacy after ruling out irritative and obstructive causes. At times incontinence is a sign of underlying disease eg TB, spinal disease, nervous disorders, parity etc. Treatment should be planned accordingly. Unfortunately majority of cases are under evaluated and emperically treated without basic information like bladder diary and pinpointing underlying cause.... Read more
