Insulin resistance means the fat and muscle cells no longer respond to the amount of insulin secreted by the pancreas. That is the insulin naturally secreted by the body is not enough to bring the blood glucose down. Before giving insulin shots we first try to control the glucose by giving insulin sensitizer like metformin or oral hypoglycemic meds like sulfonylureas. When this strategy fails we go for insulin shots which brings down the blood glucose.
